One testosterone number is not a workup

Most men who come in have had a single total testosterone drawn, been told it is within range, and left with no explanation for how they feel. Total testosterone says nothing about how much is free and usable, how much is converting to estradiol, whether the signal from the brain is intact, or whether insulin resistance and thyroid function are suppressing the whole system. We test all of it, then build the plan from what the results actually show.

Common questions

What are the signs of low testosterone?

Persistent fatigue that sleep does not fix, loss of morning erections, low libido, difficulty adding or holding muscle, weight settling around the midsection, irritability or flatness of mood, poor recovery from training, and brain fog. Most men notice several of these together over a year or two rather than one dramatic change.

What testing do you run for men?

A full hormone panel rather than a single total testosterone value: total and free testosterone, SHBG, estradiol, DHEA-S, LH and FSH, plus thyroid, fasting insulin, hemoglobin A1c, inflammatory markers, and a full nutrient picture. Testosterone almost never drops in isolation, and treating the number without the context rarely holds.

Can testosterone be raised naturally?

Frequently, yes. Insulin resistance, poor sleep, chronic stress load, low vitamin D and zinc, excess body fat, alcohol, and toxic burden all suppress production. When testing identifies which of those apply to you, correcting them can move numbers meaningfully. Testing tells us whether your case is a production problem, a conversion problem, or a lifestyle-driven suppression.

Why does estrogen matter for men?

Testosterone converts to estradiol through the aromatase enzyme, and that conversion runs higher with more body fat, higher insulin, and higher alcohol intake. A man can have adequate total testosterone and still feel terrible because too much of it is converting. That is why we measure estradiol and SHBG rather than testosterone alone.
