We are required by law to maintain the privacy of your health information, provide you with this notice of our legal duties and privacy practices, and follow the terms currently in effect.

We may use and disclose your health information for treatment, payment and health care operations. For example, we may share your information with other providers involved in your care, with laboratories that process your testing, and with vendors who help operate our clinic under confidentiality agreements.

Uses That Require Your Authorization
Uses and disclosures not described in this notice will be made only with your written authorization. You may revoke that authorization in writing at any time, except to the extent we have already acted on it.
Advertising Measurement
When you submit our contact form, we may share your email address and phone number with Google in a one-way hashed format (SHA-256) so Google can better measure which ads lead to patient inquiries. This is called "Enhanced Conversions." The hashed values cannot be read back into plain text, and we do not share any health information, symptoms, or diagnosis details with advertising platforms.
Changes to This Notice
We reserve the right to change this notice and to make the revised notice effective for information we already have as well as information we receive in the future. The most current notice will be available in our clinic and on this page.
